New Patient Registration

We are currently accepting new patients on alternating weeks with Culloden Medical Practice with whom we share our building. 

If you reside within our practice boundary (see map) and are looking to register, we require both a Registration Form and a New Patient Questionnaire to be completed for each new patient as well as photo ID and proof of address for each adult.

You can either come into the practice for these forms or find them below. These can be handed into the practice in person or they can be emailed to nhsh.gp55751-admin@nhs.scot - please ensure you have included all documents as listed in bold above.
